I authorize the Media Factory the Humane Society of Chittenden County and to record and edit into Programs/Series/Segments and related materials, my name, likeness, image, voice, performance, and submitted content. The Media Factory may use all parts of the Program, including the recordings, for any purpose and dispose of without limitation as their sole discretion determines, including airing on public television, distrubtion online and for use in the promotion of future programs.

I understand that the content I produce for the Paws and Play pet video campaign is my own and I authorize the non-exclusive distribution of this content by the Media Factory and the Humane Society of Chittenden County on my behalf. This authorization is without date restriction.
